I understand the head and gloves on the Idolarty but whats with the weird sack on his chest?

The hello kitty head is an attachment to his neck with stitches. The big thing is his real head but its sewed up so no one sees it. If he gets angry the stitches rip open and he has a mouth full of sharp teeth concealed in there but no eyes.
